GHR Search is Hiring a LPN Care Coordinator Near Harleysville, PA

A community in the Greater Harleysville Area is seeking a LPN Personal Care Coordinator to join their team! This organization is known for providing their residents with supportive living services including personal care, health care, and memory care. They also have a great reputation for being a community with a 5-star CMS rating.

Responsibilities:

As the Personal Care Coordinator, you will utilize your knowledge of nursing, established practices and organizational policies to perform various duties to coordinate the care services for residents to ensure quality care for residents and provides direction to assigned staff

  • Oversee the Unit Clerk, nurses, CNA’s and resident assistants & collaborate with the Social Worker and Life Enrichment Coordinator
  • Oversee documentation of all resident medical records to provide acceptable information for annual survey
  • Coordinate record keeping regulations regarding psychotropic drugs, restraints, resident rights, etc
  • Schedule appointments for physicians
  • Participate in the QA process
  • Communicate resident's doctor, consultants, and/or family as necessary

Schedule: Monday – Friday, 8a-430p

Salary: $73,000-$80,000

Qualifications:

  • Personal Care experience is required
  • Previous supervisory experience is required (oversee clerks, nurses, CNA’s and resident assistants)
  • 2 years of nursing experience
  • LPN or RN required

Job Type: Full-time

Pay: $70,000.00 - $80,000.00 per year
